Participate in a comprehensive, ASAM-aligned clinical evaluation utilizing evidence-based practices with a highly qualified licensed expert clinician.

Receive Comprehensive Assessment
Obtain a professional assessment report with clinical recommendations. We also assist with securing treatment and communicating with authorized legal professionals, employers and loved ones.
Sozo Recovery Care operates on a private-pay basis. Chemical health assessments are $200, with payment collected at the time of service.
Following your completed assessment, you will receive a superbill containing the information needed to request reimbursement from your insurance provider. Reimbursement is determined by your insurance plan and is not guaranteed. Sozo does not submit insurance claims directly.
